The following information is required before a student may enter the professional phase of the Athletic Training Education Program.

2. Log of Directed Observation/Athletic Training Student Hours (Use "Hours of Documentation Form")
3. Directed Observer Evaluation Form
4. ADA Technical Standards Form
5. Physical Exam Form
6. CPR First Aid Certification Card
7. Transcripts
8. Immunization Records (See Application form or Catalog for details on what immunizations are needed.)
