Please fill out the following form to register each artwork and student for participation in the College Preparatory Chaimpionships Art Competion. An individual student may have from one to three entries (if the student has three entries their name would be listed three times). An individual School may not submit more than a total of 20 art entries. This form should list every students' name participating in the Art portion of the College Preparatory Championships (Grades 9-12 only).


An individual entry form should also be downloaded, filled out and attached to each piece of art for judging purposes.

REGISTRATION FEE DUE FOR ART COMPETITION:
THERE IS ONE $12 FEE (AFTER OCTOBER 12, $15) PER STUDENT FOR PARTICIAPTION IN ANY OR ALL PARTS OF THE COLLEGE PREPARATORY CHAMPIONSHIPS. IF THE STUDENT IS PARTICIPATING IN THE ART COMPETION ONLY, PLEASE SEND THE $12 FEE (per student; not per entry). MAKE CHECKS PAYABLE TO SBU AND
